- Evidence of suicide risk assessment in the initial evaluation
- Evidence of inclusion of ECT treatment in the treatment plan for members who are diagnosed with chronic or severe depression who prefer ECT treatment or members with severe depression and any of the following features:
- Patient preference
- Previous preferential response
- Need of rapid response
- Intolerance to medication
- Evidence members have been informed of the potential side effects of medication
